How to Refer to us

To make an appointment with us, you will need a referral from a GP or Optometrist.
Please download and print the following referral pad and take it to your GP or Optometrist:

• Referral pad – For Patients, GP’s and Optometrists, 2 pages (Referral and Map/Location)

For your appointment

For your appointment, please bring:

• Current glasses and list of medications
• Medicare / Pension / DVA / Health Fund cards
• Driving is not recommended for 1-2 hours after pupil dilation
• Credit card / EFTPOS facilities available
